Admissions

Welcome to Emberwood Center!

We would like to take this opportunity to tell you a little bit about our agency and what to expect if you are considering receiving services.  Emberwood Center is a non-profit agency for all mental health issues specializing in addictions (substances and gambling) and trauma. All services are provided in an outpatient setting, and referrals can be made for additional client needs. We are certified by CARF (Commission on Accreditation of Rehabilitation Facilities) which upholds us to high standards of service delivery as well as staff competency and policies; we are contracted by the State of Indiana and we receive a grant from the Indiana Criminal Justice Institute to provide free services to victims of crimes.   Using evidence-based practices (EBP) we ensure the most effective and efficient approach to assisting individuals and families on their path to recovery.

What to Expect

Prior to meeting with a counselor, you will need to fill out a client enrollment form including some health information, billing and consent forms, and screening tool(s). At your first appointment, you will review an orientation to get familiarized with our services, processes, policies, and your rights and responsibilities.

During your assessment appointment with a counselor, you will have the opportunity to share information related to your specific situation. The counselor will ask questions to explore different aspects of your life and how it may be related to the reason you came to Emberwood Center. Following the assessment, the counselor along with our multi-disciplinary team will determine if you meet criteria for our treatment services, and make a recommendation for additional follow-up, if necessary. Treatment services may include individual, group and/or family services, or referrals to other service providers, and will be based on your individual situation.

We understand that you may be seeking services from Emberwood Center on your own, pressured into coming by friends and/or family, or you may be referred for services through a court system or other service provider. Each person’s recovery is unique, just like each person is unique. We ask that you keep an open mind and provide truthful information, as we cannot be helpful without it. We at Emberwood Center look forward to the opportunity to serve you and help you meet your goals.

If at any point in time you have questions, please feel free to ask a member of our staff. We are here for you!

Sincerely,

Emberwood Center Team
